As a courtesy to our patients, we accept and file with most all dental insurance companies. If you have questions about your specific policy, please call our office. 

Payment Options

We accept payment in the form of cash, check, debit, and credit card (Visa, Mastercard, Discover). We also accept payment from multiple types of health savings accounts. As a financing option, we accept Care Credit. Please call our office if you have questions about any of this.

Click on the New Patient Forms Image to the right to download the 3 forms you will need to bring to your first appointment. Simply print them off, fill them out and bring them with you to your first appointment.
